This is the new form required by BSA. Troop 1000 activities require all 3 sections, A, B and C for every scout and adult.

This form replaces the Class 1, 2 and 3 medical forms that were previously medical forms and should be used in their place as the older forms expire.

REQUIRED: This (MAF)form replaces all previous forms authorizing the administration of non-prescription medication to youth during troop activities. This (MAF) form or the Troop 1000 Medicine Form substituting the medications listed on the (MAF) form must be signed before any youth may participate in Troop 1000 activities
