Forms listed as 2SIDED require the user to do the following:
  • print one side,
  • then re-insert the page into the printer and print the second side.

  • For example: failure to follow this process with a health form may require a second trip to the doctor's office with the correct form.

    REQUIRED: This (MAF)form replaces all previous forms authorizing the administration of non-prescription medication to youth during troop activities. This (MAF) form or the Troop 1000 Medicine Form substituting the medications listed on the (MAF) form must be signed before any youth may participate in Troop 1000 activities

    REQUIRED:. These requirements are more stringent than those BSA demands, but with the strenuous activities engaged in by both adults and scout in Troop 1000, we require every Scout and Adult to have a completed and up-to-date Class 3 form on file with the Troop Vet
