Forms listed as 2SIDED require the user to do the following:

For example: failure to follow this process with a health form may require a second trip to the doctor's office with the correct form.

This (MAF) form or the Troop 1000 Medicine Form substituting the medications listed on the (MAF) form must be signed before any youth may participate in Troop 1000 activities
